About our PCN

Neston and Willaston Primary Care Network (PCN) was established in 2019 and is one of the smallest PCN’s in the country covering 21 thousand patients. The network is a collaboration of Neston Surgery, Neston Medical Centre and Willaston Surgery who work closely with the community with the aim of creating a healthier future for our patient population. Within the PCN we have a dedicated team involved with initial planning, sourcing, funding and the implementation of these much-needed services.

The PCN has already established roles including Visiting Paramedics, Physiotherapists, Social Prescribing Link Workers, a Clinical Pharmacy team and a counselling service.

Our Vision is to:

  • Enable patients to see the correct Health Care Professional through effective care navigation
  • Provide a consistent level of care across the three practices
  • Encourage team working and a supportive and welcoming working environment

Our Values are:

  • We work collaboratively for the patients within the PCN
  • We are committed to providing a high level of care to our patients
  • Patient care is at the heart of everything we do
  • We always show respect and dignity and sensitivity to both our patients and colleagues
  • We provide a positive and welcoming working environment
  • We are committed to improving the lives of all patients within our PCN

Our Mission is to:

  • Work collaboratively as practices and with our community partners to provide high quality care to our patient population
