New Patients

We are using a new online service called Online Registration that makes it easy to register with this GP surgery.   .

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register. Please follow the link to start http://gp-registration.nhs.uk/N81125

Patient Access

If you do not wish to register as a patient within the NHS App,  you will be required to complete the following:

• 3 registration forms by clicking here 
• provide 2 forms of ID, 1 photographic and 1 proof of address
• attend the practice for a new patient health check which can be booked as soon as you have received confirmation of your registration

Please note you will not be registered until two forms of ID have been provided to reception.

Nominated Pharmacy

All new patients will be requested to nominate a pharmacy, this enables prescriptions to be sent electronically. Please state your preference when completing the registration forms.

Named Accountable GP

The Practice is required to provide a named accountable GP for all patients. Having a named accountable GP does not prevent you from seeing a GP of your choice.

Summary Care Record

By completing the new patient registration forms you have consented to sharing your medical records with other health care professionals. This enables your medical history and medications to be seen by hospital clinicians and other health care professionals should you require additional care outside of your GP practice. Your record will not be shared outside of the NHS, if you wish to opt out please contact reception.
